Hmm, I *could* do that contractor job (re: knowledge and access), but I wouldn't touch it with a long pole, regardless of who suggested it. It's not realistic. I think the ISP risks are much lower than you seem to think; however, it's not right for you to have the burden of providing contracts and equipment in the scenario as outlined. If the hiring person wants a small IT outpost, the hiring person needs to furnish that outpost with required resources, both physical and legal.
It . . . kind of doesn't matter whether the doctor is pleased with your progress, if the doctor isn't aware that this stuff takes time. (It's been only several months, yes?)
